The provider limit is who's actively receiving signals at the same time: a Small office fits 3 providers on shift at once, a Large office 6. Everyone can have the app installed; if a 4th provider signs on shift they just swap with someone who's finished for the day. Clinical-staff ceilings scale to how your specialty actually staffs (a dentist runs more chairs than a physical therapist runs tables), which is what the specialty picker above the plans shows. The caps are deliberately roomy, so a normally staffed office never hits them.
